Short survey will help Prince Edward Family Health Team build services

The Prince Edward Family Health Team’s vision for County residents is that they have the best possible health throughout life. Its mission is to be partners in providing high quality person-centred integrated primary care.

To ensure the PEFHT is doing everything it can to meet these goals, it is reaching out to the community to share, gather and exchange information to build healthcare services that meet Prince Edward County needs… together.

First things first – a starting point to find out about what the community knows about the PEFHT, and how it can best connect with you. This short survey will help tell them about that.

It should take about five minutes to finish. There are no right or wrong answers. The PEFHT appreciates information from any and all who would like to provide it. It’s anonymous, but if you would like to chat more, there’s a spot to add your name and contact information.
